Quarterly-retest reminder and re-engagement loop

The retest is the whole membership — it is the lab draw that earns the next consult and proves the program is working. The agent makes sure the window does not slip by quietly.

What it does

On a hormone or longevity membership the recurring retest is the loop the whole program turns on: a blood draw every quarter feeds the consult that refines the plan and justifies the renewal. When that window slips, nothing visibly breaks, so nobody notices — the patient keeps the subscription but stops generating the data, the consult has nothing to act on, and a few quiet months later they cancel a membership that stopped meaning anything. The agent treats the retest date as the heartbeat: it fires the booking nudge when the window opens, chases the patient who lets it drift, and routes a completed draw into the clinician consult so the loop closes instead of fading.

How it works

  1. 1
Trigger. a time signal that a patient's quarterly retest window has opened on the membership calendar, or a behavioral signal that the window is closing with no kit ordered and no draw booked.
  2. 2
Decision. the agent reads memory for the last retest date, whether an at-home kit or a phlebotomy slot is the patient's usual route, and engagement state, then decides between a first booking nudge, a softer chase for a slipping window, or a re-engagement reach for someone who has gone quiet across a cycle.
  3. 3
Action. it sends the retest nudge on the channel the patient answers — order the at-home kit or book the draw, in one tap — and frames it as the step that unlocks the next consult, not as admin.
  4. 4
Follow-through. a completed draw and returned results write back to memory and open the clinician consult; the agent never reads the values itself. If the patient already booked, the queued chase is cancelled before it fires; a window that lapses past the limit, or a patient silent across a full cycle, escalates to the care team as a retention risk. Lift is measured against a holdout.
Example
AgentYour next retest window is open — this is the blood panel that sets up your quarterly consult and shows what's moved since last time. Want me to send your at-home kit now, or book a draw near you?
PatientSend the kit. I keep meaning to and forgetting.
AgentDone — your kit is on the way with the return label inside. Once your sample is in, your results flow straight to your clinician for the consult, and I'll let you know when they're ready. I'll check in if it hasn't shipped back so the window doesn't close on you.

Configuration

How the agent is wired for this use case.

Triggera retest_window_open time signal from the membership / scheduling system on the patient's quarterly cadence, or a window_closing behavioral signal with no kit ordered or draw booked.
Tools & actions
  • Scheduling system · read the retest cadence and window, book or hold a phlebotomy slot.
  • Lab system · order the at-home kit, track sample return, and receive completed results.
  • Patient memory store · read last retest date, preferred route, and engagement state; write back completed draws and re-engagement state.
  • Messaging channel · send the booking nudge, the slipping-window chase, and the re-engagement reach on push, email, SMS, or in-app.
  • Clinician consult queue · open the consult once results return; the agent surfaces, it does not interpret.
  • Care-team escalation path · flag a lapsed window or a cross-cycle silent patient as a retention risk.
Autonomyretest nudges, slipping-window chases, and kit orders run unattended behind judge gating with the booking prepared for one tap; the agent moves the logistics of the draw only — it never reads, scores, or interprets a result and never touches a dose, with results routed to the clinician (detect the lapse and surface, do not interpret).
Channelspush · email · sms · in-app
Escalationa retest window that lapses past the program limit, or a patient silent across a full cycle, hands off to the care team as a retention risk; a returned result routes to the clinician for the consult.

What you need

The inputs this use case runs on. Your channels stay yours; the agent supplies the judgment.

Signals

the quarterly retest-window-open event, the window-closing event, kit-ordered and sample-returned events, patient-initiated bookings for cancellation.

Data

retest cadence and last draw date, preferred kit-versus-phlebotomy route, membership and renewal state, engagement history, patient memory, channel consent.

Guardrails

the agent never interprets a lab value or advises on a dose — results route to the clinician; judge gating on every send; frequency caps; stale-chase cancellation when the patient self-books; holdout measurement.

Metrics it moves

  • renewal-rateup, because a membership that keeps completing its retest keeps producing the data and the consult that make renewal worth it.
  • adherenceup, the recurring draw that the whole protocol depends on actually happens on cadence instead of drifting.
  • ltvup, by catching the silent retest lapse that precedes a quiet cancellation and re-engaging the patient before they go.
